Payment required for any of the following can be made by check or by using the “MuniciPay” link to right to pay online. If paying online, be sure to include your name and the permit/form type so payment can be applied correctly. Any payments not clearly labeled will be refunded.

Birth and Death Certificate Request Form 

• This form can be used to request birth or death certificates for anyone born or who passed in the State of Vermont. Fill out the form and return it to the Clerk, along with a fee of $10.00 per certified copy requested. These certificates can be gotten from any Town Clerk in the state–you do not need to request it from the town in which the birth or death occurred.
• Instead of using this form, you can visit https://secure.vermont.gov/VSARA/vitalrecords/ to order a birth or death certificate completely online and have it mailed to you anywhere in the US. Most people find this option quicker and easier if they are not currently located near a Vermont town clerk’s office.

 

Zoning Permit

• This form serves as the permit for any building, addition, subdivision, or driveway creation/alteration.
• Fees:
  • For the first building on a property: $57.00
  • For subsequent buildings, additions, decks/porches, or subdivisons: $27.00
  • For driveways: $0.00
